What to Bring:
Certified Copy of Child’s Birth Certificate
 Social Security Card
 Proof of Residency (Electric Bill or any utility bill, NO CELL
PHONE BILL)
 If your child is 4 years old at the time of registration, a record of
the eye, ear, and dental examination and Georgia Certificate of
Immunization is required.
